*Effective in summer 2025, the Integrated Therapy Unit (ITU) will be relocated to our Children’s Health Plano campus. New hires must be committed to working at our current Dallas campus location until the transition to Plano is complete.

Summary:

As a Patient & Family Educator, you will provide comprehensive, individualized education to children and their families to include planning, coordinating, implementing, and evaluating their learning to aid in successful transition to home and meet ongoing self-care management needs. This role requires in-depth professional knowledge and practical/applied expertise in own discipline and basic knowledge of related disciplines within the broader professional field. A successful candidate will have knowledge of best practices and how own area integrates with others; demonstrates awareness of the industry, including regulatory, evolving customer demands, and the factors that differentiate the organization in the market.

Job Highlights include but are not limited to:

  • Utilize the knowledge of educational theory to promote health & wellness among patients and families.
  • Assess the educational needs of the patient and caregivers and form an education plan for safe care.
  • Collaborate with patient, family and interdisciplinary care team to establish goals for self-management skills training and education.
  • Review pertinent patient data (laboratory tests, diagnoses, home skills needed, medical history, diagnostic procedural results, etc.) to aid in developing education plan.
  •  Provide initial and continuing education and skills training for children and caregivers on the care that will be required upon discharge from the hospital and across the care continuum. 
  • Educate and mentor other disciplines around the impact of health literacy on patient care and outcomes.
  • Demonstrate the knowledge and skills necessary to provide care appropriate to patients ranging in age from 0 to 18 years.
  •  Possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.
  • Provide the care needed as described in the unit's policies and procedures.
  • Comply with state, federal and other regulations.

Other information:

WORK EXPERIENCE

  •  At least 2 years' experience working as a rehabilitation nurse preferred.
  • Pediatric experience strongly preferred.

EDUCATION

  • BSN required.

LICENSES AND CERTIFICATIONS
  • Current license to practice professional nursing in the State of Texas required.
  • BLS certification approved by the American Heart Association required.
  • PALS/NRP/ACLS (Department specific requirements - see addendum) required.
  • Effective April 20, 2020, new employees must obtain PACES certification within 6 months of hire and existing employees must obtain PACES certification upon expiration of their PEARS certification (no later than April 20, 2022). (Department specific requirement - see addendum) Req
